Using this Website does not create a doctor-patient relationship between you and Soma Chiropractic, Dr. Cid Correa, or any member of our team. A doctor-patient relationship is established only after you attend an in-person consultation and examination at our office and we agree to accept you as a patient.

2. Appointment requests and scheduling

 

Any appointment request, contact form, or booking submitted through the Website is a request only. It is not a confirmed appointment.

An appointment is confirmed only when our team contacts you directly to confirm it, or when you receive a confirmation from us by email or text message. We are not able to guarantee the availability of any particular date or time, and we may need to reschedule.

 

The Website is not a channel for urgent or emergency communication. Do not use the Website, our contact forms, email, or text messages to communicate anything time-sensitive or urgent. If you or your child are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.

3. Please do not send health information through the Website

 

Our contact and appointment request forms are for scheduling and general inquiries. Please do not submit detailed health or medical information through the Website, including diagnoses, symptoms, test results, medications, or information about your child's medical history.

Information you submit through the Website is transmitted over the internet and is not encrypted end-to-end. If you need to share health information with us, we will provide a secure method during your intake or at your visit.

 

Health information you provide to us as a patient, during intake, examination, or the course of care, becomes part of your confidential patient record, protected under Florida law and our professional obligations. It is not governed by these Terms. Information you provide as a Website visitor is governed by our Privacy Policy. These are separate categories of information handled under separate rules.

If you provide your phone number or email address, you agree that we may contact you at that number or address regarding your inquiry, appointment requests, and scheduling.

 

Appointment reminders and promotional messages are separate. We obtain your consent separately for each. You may opt out of text messages at any time by replying STOP, and you may opt out of promotional emails using the unsubscribe link in any such email. Opting out of promotional messages does not opt you out of essential communications about your care or appointments.

 

Message and data rates may apply. Message frequency varies.
